How much is a session at Gradus?
At Gradus, payment for is due at the time of service. If your insurance has out-of-network benefits, our administrative staff will submit claims on your behalf, and any reimbursement will go directly to you. Do I need a prescription to receive care at Gradus?
New York is a direct access state, meaning you can begin physical therapy without a physician’s prescription. Additionally, some insurance plans may still require a referral for reimbursement, which our administrative team can verify on your behalf.

What should I expect during my first visit?
Your initial visit will focus on understanding your current condition through a detailed discussion and review of your history, followed by a thorough physical examination, hands-on treatment, and a personalized home exercise plan.

What do I need to bring to my appointment?
Please ensure that your photo ID and insurance card have already been sent to our administrative staff prior to your visit at info@graduspt.com. If possible, complete the registration form before your visit, or plan to arrive 15–20 minutes early to fill it out. Please bring all relevant written medical records (MRI, CT scan, X-ray, nerve conduction tests, and post-operative reports), and a list of your current medications. Wear loose, comfortable clothing that allows us easy access to the areas you’d like assessed.
